You must complete all sections of the form as required. This information will be assessed by our team and we reserve the right to ask for a medical assessment by a qualified physician.

It is important to always consult with a physician prior to departing on an adventure or expedition style trip to confirm that you are physically fit for travel. This is especially important as we will be travelling in remote areas where immediate help may be slow to arrive, and medical facilities may be basic or non existent. Please confirm with your medical professional that your are medically fit for travel prior to departure. 

We reserve the right to deny departure if you fail to disclose any medical conditions that may affect your safety, or the safety of others with no compensation. Refusal or non-completion of this medical form will result in immediate cancellation of your booking in accordance with the cancellation conditions of GF International LTD’s terms and conditions.

It is a requirement prior to departure that this form is completed with complete, accurate and updated information. If there are any changes to your physical/medical condition you must notify us prior to departure.
